Kerrigan & Holt is an outpatient physical therapy practice with two locations in Boulder County, Colorado. We treat shoulders, knees, hips, backs, concussions, and vestibular dysfunction — and we treat them by actually treating them: 55-minute one-on-one sessions with a Doctor of Physical Therapy, not 18 minutes ping-ponging between techs. Every patient at Kerrigan & Holt completes validated outcome measures at intake, at week 4, and at discharge. The measures depend on the condition: LEFS for lower-extremity, QuickDASH for upper-extremity, NDI for neck, ODI for low-back, ABC for balance, VOMS for concussion. What patients ask
at evaluation.

Why 55 minutes 1:1? Most PT clinics don't do that.

Because PT that works requires your therapist's attention for more than 6 minutes at a time. The industry standard — particularly at clinics owned by hospital systems or PE-backed chains — is 2-4 patients per therapist per hour, managed between aides and students. Those models collect more insurance dollars per therapist hour; they don't produce better outcomes. Our model costs us ~22% more to staff; it's why our contracted rate is slightly higher and our cash rate too. It's also why our visit counts are 40% lower than industry average and our outcomes are 16 percentage points above the FOTO benchmark.

How many visits will I need?

Honest answer: we don't know at intake, but we can usually estimate within 2 visits. Averages by condition (our data Q3 2025): low back pain 6-9 visits; knee OA 8-12; post-op ACL 28-36 across 5 months; concussion/vestibular 4-6; rotator cuff repair 14-18; pelvic pain 8-14. We set a visit budget at your evaluation and re-check against outcomes every 4 visits. If we're not progressing, we change the plan or refer out — we will never keep you longer than you need.

What if I have Kaiser or a narrow HMO?

Kaiser Permanente and some narrow HMOs require in-network PT. We're contracted with Kaiser as a specialty-referral provider for pelvic health and vestibular (specialties they don't offer in-house). For general ortho, Kaiser members typically start in the Kaiser PT department; we happily see Kaiser patients as out-of-network for sports performance assessments or second opinions. Cash rate applies; we provide superbills but Kaiser does not reimburse out-of-network PT.

Will you work with my surgeon?

Yes — we send weekly progress notes to every surgeon whose patient is in our care, and we'll call them directly if anything looks off. We work most frequently with: Boulder Orthopedic & Spine (Drs. Patel, Maclean, Brogan), Panorama Orthopedics (Drs. Kim, Okafor), Mapleton Hill Ortho, CU Sports Medicine, and Colorado Brain & Spine. If your surgeon wants a specific protocol, we follow it exactly; if they're open to our standard protocols, our outcomes tend to be better with more strength work than most post-op protocols prescribe.

Is dry needling part of this?

Yes, offered by 7 of our 12 DPTs, all with FDN-1 or Spinal Manipulative Therapy certification and Colorado Board authorization. Dry needling is used for trigger-point release, chronic tendinopathy, and chronic pain — it is not acupuncture and we don't claim it is. Evidence for dry needling is strongest for short-term pain reduction combined with exercise; we use it that way. Add-on cost: $40 per session for cash patients, covered under CPT 20560/20561 for most PPO plans.

Can I just do the exercises at home?

Often yes — and that's fine with us. At evaluation we'll tell you honestly whether you're likely to do well with 2-3 home visits and a written program, vs 8-12 in-clinic visits. Roughly 25% of our eval patients get a "do this at home for 6 weeks, come back if you're not better" plan. We'd rather you heal than collect billables.
